ESM - hanging on to the scalpel?

This may sound like an off-the-wall question, but if I explain why it'll be too much of a spoiler.

Basically: Do you have to lose the scalpel in order to (indirectly) get the last reagent? I can't think of anything else I've been putting off that could trigger a sports event.
