I've heard tell that cruise lines use their newest and shiniest ships for 7+ day cruises, and these ships are very well cared for.

The older, less good ships are used for 3-4 day cruises, and for some reason these ships are unloved by their owners.

Considering that this fiasco apparently happened on a 4 day cruise, this may be part of the problem. But there's also the possibility that it was a fourth-rate cruise line.
