Not only have I done that, I was in the marching band picture (labeled as band captain, no less) with my band jacket buttoned wrong (fortunately, the pictures were reduced so much in the yearbook no one could tell).
But worse that that is the time I went all day with my shirt on inside-out *and* backwards - yes, tag sticking out below my neck - and no one told me. I didn't realize it until I changed into my pajamas that night.

Yep. And there is just no graceful way to ask "hey, by the way, what's your name?" of someone who obviously expects you to know him. (I ask anyway. It's just not graceful.)
