I've never heard of it before, but I love the words to that hymn. Unfortunately, I can't seem to find any music for it online that matches your description. I can remember learning this hymn when I was about 10 for the school choir. We had to sing at an ANZAC ceremony and it was very moving. I can still remember the tune...
