I use any of the three methods you mentioned, depending on the circumstances. In this post, I'm replying to a question or problem you posed, so I've left the section of the previous post in which you posed your question as a reference point.
When someone poses several questions/problems within a single post, I'll often use the "insert new material in between the lines" method.
I generally don't remove all of the previous post from my reply unless I'm typing a very specific response for a specific person. Because I know that that person knows what they just wrote and doesn't need a reminder. And maybe sometimes when I'm posting a completely separate thought under an already-established thread. But I don't think I do that very often.
And sometimes I probably just use one of the three methods at random, based on my whim of the moment.
