The chain of events went approximately like this:
The Scrabble tile (home-made flat rock style, since this was on Earth a million years ago or so) startled a rabbit, which ran off and thereby was caught by a fox, which choked on one of its bones. The fox died and fell into a pond, which eventually was poisoned by the rotting animal. Ford's girlfriend drank from that pond, and died.
